The reason for my ban is because I have a severe gambling addiction and I am currently in a GA program to help me with this addiction.

I would like this ban to be for life and I would not like it ever reversed.

Please can you also exclude me for life should you have a betting station I can walk into.

I would also like to find out about the game blocks so I receive no communication or marketing.

Please ensure this is non reversible as I would like this for LIFE.

Please could you kindly send through any other ways I can ban myself with the license provider you are registered with so no other play can be done on the other sites.
